Mary Hughes has lived, studied, and worked primarily in the Scottsdale, Tempe, and Phoenix area since 1970, and through the years has been an active participant in several Valley charities and nonprofits.
With a Bachelor's Degree in Social Work from the Arizona State University School of Social Work, a Care Management program certificate from Boston University's Center for Aging and Disability Education and Research, a certificate in facilitation from the University of Wisconsin-Madison, and 7 years prior experience as a long term care case manager/rehabilitation discharge planner, Mary is committed to providing high-quality service to the senior community and their adult children.
As principal of Arizona Senior Transitions LLC for the past 13 years, her individual practice includes private geriatric care management, and assisted living and memory care referrals.
In her practice, Mary frequently relies on and refers clients to select colleagues in the community: professionals who comprise a collaborative network of physicians, elder law attorneys, licensed fiduciaries, home care, home health, hospice, and mental health practitioners.
Her reputation as an effective, compassionate social worker is built on a deep understanding of the needs of older people and the challenges facing their adult children, as well as an awareness that, for all of us, our well-being often takes the combined effort of many caring, compassionate people.
